 You will receive a secure link to fill out forms online. You can also attach your ID card and insurance information through this link. 

TIME: Please arrive 30 minutes prior to your appointment time. Remember to bring a valid ID card and your insurance card if you have not attached them with the online form.

PAYMENT: We only accept Visa, Discover, and MasterCard credit cards. We do not accept checks or American Express cards.

If you need to provide previous treatment records, fill out the Release of Information form.
